Certified Coder
JOB OBJECTIVE: Assigns appropriate ICD-9-CM and CPT-4 codes to inpatient and outpatient records. Abstracts designated criteria for tracking, reporting and reimbursement purposes.
ESSENTIAL JOB FUNCTIONS:
1. Accurately assigns diagnoses, procedure codes and DRG’s to inpatient, day surgery, observation, outpatient, and emergency room records utilizing appropriate coding guidelines.
2. Coding is consistent with applicable regulations, including the official coding promulgated by CMS, the American Hospital Association, American Health Information Management Association and compliance policies.
3. Communicates with physicians/physician offices to clarify diagnosis or procedures for appropriate code assignment when documentation is unclear.
4. Coders are key individuals that may catch lost charges and incorrect charges that have not been entered by various departments. This information is communicated to supervisor, or responsible department. Reports coding issues to supervisor.

Qualifications:
1. High School Education or higher.
2. Certified Coder Certificate/Degree
3. Proficiency with PC software including but not limited to knowledge of Excel, Microsoft Word, Etc.
4. Must be familiar with Medical Terminology
5. Requires repetitive, bending, pulling, pushing, kneeling, walking and lifting up to 50 lbs.
6. Must be able to read and write English fluently.
7. Must be able to clearly and accurately communicate with nursing, physicians, administration and public
8. Visual acuity within normal limits. Must be able to read charts, orders, notes, and office reports.
